Health Care Access Now (HCAN) sustainably improves the health of Greater Cincinnati’s uninsured and
vulnerable residents by providing solutions to health disparities and barriers to accessible healthcare
through community-based care coordination and workforce development programs.
While HCAN does not provide direct medical care, our organization plays a critical role in improving health,
advancing equity, and eliminating health disparities and barriers to accessible healthcare for residents
throughout Greater Cincinnati. HCAN is the intermediary that empowers more than 1,000 individuals each
year to achieve self-sufficiency and lasting well-being.
HCAN:
1. leads Southwest Ohio’s Pathways Community Hub, integrating eleven local service providers and state and
national stakeholders to serve as a care coordination center for residents within a certain geography that
need support to access and maintain quality healthcare services.
2. helps pregnant, maternal, pediatric, and adult clients address the root causes of their mental, behavioral, or
physical health issues; overcome their barriers to wellness; and sustainably transform their lives for the
better through community-based care coordination programs.
3. provides career pathway opportunities by training, upskilling, and hiring from communities facing
educational inequities. Our workforce development programming includes the CHW Certification Program,
ongoing training, Supervisor Training, and (currently in development) CHW Apprenticeship Program. HCAN
also hires CHWs from the populations it serves, knowing trusted CHW/client relationships are built through
shared experiences, language, and knowledge of their unique community.
